MEDICATION AUTHORIZATION
Illinois School Code mandates that there must be a written authorization, signed by a health care provider, for any medication given at school. A Medication Authorization Form must be completed every school year for ALL medications. This includes Tylenol, Ibuprofen, Aleve, TUMS, cough syrup, cough drops, antibiotics, ear drops, prescription eye drops, etc... No medication will be given without a medication authorization signed by health care provider. If your student requires medications during school hours, please complete the form, have provider sign it and return it to school.
REQUIRED MEDICAL FORMS
State of Illinois Certificate of Child Health Examination - due by the first day of the school for all new students and students entering Preschool, Kindergarten and 6th grade.
State of Illinois Eye Examination Report - due by September 1st of the school year for all new students and students entering kindergarten.
State of Illinois Proof of Dental Examination Form - due by September 1st of the school year for students attending Kindergarten, 2nd and 6th grade.
Required Forms for 4th-8th grade students participating in sports:
IESA sports physical - expires after 395 days. A State of Illinois child health exam is accepted in place of the IESA. Physicals are due before the start of the season.
Concussion information & Concussion Signature - signature page is due before the start of season. Must be updated each school year.

Family Emergency Information Form
Medication Authorization Form
Asthma Care Plan (if applicable)
Diabetes Medical Management Plan (if applicable)
Food Allergy Emergency Action Plan (if applicable)
Seizure Action Plan (if applicable)
REQUIRED VACCINATIONS
Children must be vaccinated in alignment with the requirements set by the state of Illinois.
